Where can the employer obtain the forms for this new procedure?

 Effective from May 2008, the cities of Amsterdam and Amstelveen and the IND have launched a joint initiative. Starting 5 January 2009 Rotterdam has joined this initiative. In just one visit to the Expatcenter, highly skilled migrants can be registered in the Municipal Administration and at the same time collect their residence document. Employers can access the services of the Expatcenter by using the forms and appendices listed below.

6. Where can the employer obtain the forms for this new procedure?

The employer can download application forms from this page. He then completes the application form and the appendices on the computer, using Word. The application form and appendices, plus one copy, should be put into an envelope bearing the sticker 'Expatcenter Amsterdam' or 'Expatdesk Rotterdam' and sent to the IND at the following address:
